We exclusively prepare a plan of care for every single one of our clients. We believe that no two people are exactly alike so we make sure that the care we provide is designed to be as unique as the individual who will be receiving our home health services. Dominion Home Health Agency will closely coordinate with your physician to design a care plan that will be parallel to the prescribed treatment or rehabilitation plan.
